These instructions provide you with all of the details necessary for the Game Administrators to verify your submission. It also provides you with the form needed to complete so that your prize, once verified, can be awarded to you!


Here's all you have to do:

  1. Print off the Instant Win Statement of Compliance Release and Indemnity Agreement Form here.
  2. Complete the Instant Win Statement of Compliance Release and Indemnity Agreement Form. Carefully print all of the required information and sign the form where requested.
  3. You must correctly complete the skill-testing question.
  4. Place your original winning Instant Prize message with your completed Instant Win Statement of Compliance Release and Indemnity Agreement Form into an envelope.
  5. Mail your submission to: Casino 73, c/o Pizza 73, 500 Kipling Avenue, Toronto, ON M8Z 5E5.
    You may wish to send your completed prize submission form via registered mail for your own protection. Also please take the time to photocopy your Instant Prize message in case your submission is lost in the mail.
  6. All submissions must be postmarked by January 31, 2012.

Once verified, your prize will be mailed to you at the address provided with your submission. Allow up to four to eight weeks for redemption of the prize following verification.
